Aspire East Palo Alto Phoenix Academy Enrollment Information
Open Enrollment for 2013-14 School Year (grades 7-8): February 1 to February 28, 2013

A completed student interest form must be submitted to the main office (1039 Garden Street, East Palo Alto) between February 1, 2012 and February 28, 2012. The lottery will be held on Monday, March 4, 2013 at 10:00am. You will be notified by phone and/or mail of the lottery outcome. If your child gets a space, you will be required to submit the required documents listed below no later than March 21, 2013 in order to finalize registration process. We have a very long waitlist of student applicants; if we do not receive all required documents by the due date, your child will lose his or her space.

Required Documents List
New Applicants to Aspire Public Schools Applicants from EPACS or Other Aspire Schools
  1. Original Birth/Baptism Certificate
  2. Residence Verification
  3. Immunization Record
    • TDAP (new California requirement for 7th grade and up)
    • 4 Polio (if only 3 doses given, 3rd dose must be after 4th birthday)
    • 3 Hepatitis B
    • 5 DTP (if only 4 doses given, 4th dose must be after 4th birthday)
    • Varicella Immunization
    • 2 MMR (both doses must be on or after 1st birthday)
    • T.B. Test (Negative) if results are positive we require doctor’s note (within 3 years)
  4. Most Recent Transcript (or Report Card if applying for 7th-9th grade)
  5. CST Report (most recent STAR Test Results)
  6. Physical Exam
  1. Proof of TDAP Immunization (new California requirement for 7th grade and up)
All other information will automatically be transferred to EPAPA. If you have questions, contact the main office (650-325-1460).
